Body Procedures

Body surgery opens a wide arena of possibilities for the look you want to achieve.

To choose a Plastic Surgeon,  ask your family doctor for advice or consult with friends and acquaintances. When a name comes up several times and always with good references is usually a good sign. Remember that each case is unique and that your friend’s good results do not guarantee yours.

Blepharoplasty is a cosmetic surgery designed to remove excess and sagging skin from the upper and / or lower eyelids. As a person ages, the skin loses some of its fat and much of its elasticity, becoming saggy and wrinkled. This process, which can be accelerated by sudden weight loss, makes the eyelids protrude by herniating small pockets of fat that must be treated.

Objectives:
The removal of excess skin aims to improve the appearance of the individual:

In upper blepharoplasty, a horizontal fold of skin is removed from the center of each upper eyelid so that the scar runs along a natural fold.

In the inferior blepharoplasty, the incision is made below the lashes, so that the scar is in the shadow area of the lashes and extends obliquely, to look like an expression wrinkle.

Anesthesia
The intervention is performed with local anesthesia or general anesthesia under outpatient surgery and takes about 90 minutes.

Recovery:
After surgery, the patient will feel hot and will have bruises and swelling around the eyes. The eyes should be covered with an antibiotic cream to prevent infection. Ice is also applied to the eyes to reduce swelling and bruising. The first usually disappear after 3 days, and the bruises around two weeks. Suture points are removed 3 to 5 days after surgery and scarring becomes inoperable after 3 to 6 months in most cases.

Increase the projection of the gluteal region by injecting fat with the liposuction cannulas. There are 2 types of body shapes:

  1. The android form, with a fat concentration above the waist,
  2. Or the gynoid, the one that concentrates fat mainly in the hips and below the waist.

Both types can benefit from this type of surgery.

We can also enlarge the buttocks with the placement of intramuscular prostheses, in this case the incision is made in the buttock groove allowing a considerable and lasting increase.

Surgical procedure:
These are usually made from 3 to 6 small incisions in the hip region and buttock fold. The biggest advantage of the liposculpture technique, in this case, is the combination of excess fat aspiration and rejection in depressed regions. In most cases, there is an accumulation of fat in the hips, breeches and inner thighs, and only with the removal of excess fat from these regions will there be an improvement in body contour. This excess fat can be used to be grafted to the gluteal region. Fat is injected into the subcutaneous tissue depending on the area and specific problem to be corrected.

Duration:
1 to 3 hours.

Anesthesia:
Sedation site or general

Hospitalization / Outpatient Time:
Usually hospitalization from 24 hours to 48 hours

Recovery:
Postoperatively we prescribe the use of an elastic strap that will help maintain the new shape after the increase. Lymphatic drainage of the aspirated areas helps absorb swelling, but it is best to avoid over the injected part. It may be started 24 hours postoperatively. Walking 1.5 kilometers is essential after day 3 and should increase the distance traveled daily. From the 3rd. week, low-frequency ultrasound and endermology will help correct any stiffened areas. This postoperative procedure should be done by a trained beautician or physiotherapist and under the supervision of the surgeon. Strong exercises like running and weight lifting are allowed after 30 days. Mild temporary pain, especially in the first 48 hours can be treated with pain killers. Swelling, discomfort, numbness and skin pigmentation usually occur within the first few weeks. Anesthetic fluid will escape from the incisions within the first 24 hours. The dressings will be changed daily. Bath only after 24 hours. You should wear the strap for a few weeks. The dressings will be removed between the 5th and 7th day. Back to work: 2-3 weeks. Gymnastics and sunbathing: 4 to 6 weeks or more.

Complications:
In liposculpture complications are not frequent. Complications may occur as to symmetry, curl and pigmentation of the skin, discomfort and problems with anesthesia. Temporary numbness of the skin, bruising and bruising may occur. Skin irregularities may occur in some patients, but if necessary, may be treated. More serious complications involve fluid or blood loss that can cause a shock, but it is rare for tumescent anesthesia. Infections are also very rare. A few patients are disappointed with the fat injection technique when resorption is moderate (between 50 – 60%) or because they do not have enough fat to reach the desired size. We can inject 500ml of fat into each buttock depending on the case, injected into the muscle, subcutaneously until it reaches the required amount of fat. The process can be repeated if necessary.

Details:
Liposuction improves body contour and self-esteem. Results will appear within a few months as there is a possibility of fluid retention after surgery with prolonged swelling. Sometimes the fat injection procedure needs to be redone because there is a limit to the amount of fat that can be injected in a single stage. Occasionally, a second procedure, or “retouching” will be more appropriate to improve certain areas, usually with anesthetic

Results duration:
After the 6 month no further resorption should occur and the result should remain the same. If body weight is maintained, the results will be permanent, especially if combined with diet and exercise.

The Cervico-Facial Lifting is intended to soften the signs of aging (wrinkles and expression marks) caused by the action of time. It is most suitable for those people who have wrinkles, sagging or excess skin on the face.

Length of stay:
is about 24 hours and the patient should try to have a slightly elevated head when resting.

Postoperative:
Compressive dressings should be made within the first 72 hours. Sun exposure should be avoided for the first thirty days after this period with caution and the use of sunscreen.

Candidates:
Usually men and women over 40

Objective:
Improve the deep groove that goes from the nose to the corner of the mouth, the cheeks, the jawline, and the cheekbones. It also helps to improve sagging skin, puffiness and sagging neck skin by removing excess skin, correcting muscles and reshaping the skin. It can be done in conjunction with frontal liting and blepharoplasty to correct excess eyebrows and puffiness of the eyebrows.

Incisions and technique:
The incision is usually placed in front and behind the ear, hidden by the natural ridges. Behind the ear, the incision enters the scalp, hidden by the hair. The earlobe should be loose and natural, avoiding the taut appearance. Sometimes a small incision under the chin can be used to help correct the neck muscles (platysma). Preferably the deep structures of the face and neck will also be repositioned. The skin is pulled up and back and the excess is removed. The appearance should look normal and not stretched. To correct the neck liposculpture can also be used, improving the contour. One of the most important aspects of facial surgery is the replacement of the lost volume. This can be achieved either by folding and suturing deep structures or by injecting fat from another region.

Duration:
Average of 3 to 4 hours.

Anesthesia:
Local with sedation or general.

Inpatient / Outpatient:
Most patients need 24 hours of hospitalization

Recovery:
Back to work: 10 to 14 days. More strenuous physical activities: Three weeks Bruises: 2 to 3 weeks. Avoid sun exposure for two months.

Myths and Realities:
The unusual acceptance and expansion of cosmetic surgery in the last two decades have its genesis in social and individual motivations.

Among the social motivations: the aging of the population with the increase in life expectancy, the professional exclusion of geronts, the fashion and the enslaving media that impose biotypes and silhouettes depersonalizing the ego, the overvaluation of youth with their hedonistic cult, the delusional. Obsession with the politically cultivated image is the most relevant social factor contributing to this collective desire for eternal adolescence.

Secondly, individual motivations: the pace of biological degradation, usually faster than psychological deterioration, results in a lagged and aging real image of self-consciousness, the need to compete professionally and sexually in order to feel a new object. of desire and finally the refusal of the idea of death

We should focus only on those that we consider being the most effective and lasting techniques to attenuate and counteract the marks of facial aging – cervico-facial rhytidoplasty and blepharoplasty showing clinical cases from our experience addressing the appropriate choice for each patient and each face. as well as the details to avoid iatrogenic sequelae and stigmas.

Attention should be given to unrealistic expectations with thorough clarification eliminating risks such as large smokers and psychiatric disorders.

Finally, we must talk about the main complications and disappointments of these interventions from the point of view of the surgeon and the patient.

The longevity of our species has its genetically predetermined biological limit (Telomeres), but there is no consensus theory that explains aging that makes us all a being for death.

Until a factor or set of factors can act upon this relentless biological clock, Aesthetic Surgery is a legitimate transitional solution for living better with what we are becoming.

Risks:
Complications are not frequent. Hematoma (accumulation of blood under the skin), infection, bleeding and reactions to anesthesia. Some incisions may be visible due to variations in healing and may need to be retouched. Temporary injury to nerves controlling the muscles of the face or tenderness is rarely permanent. Asymmetry and change in the hair implantation line.

Details:
This intervention does not correct wrinkles around the mouth. The choice of technique depends on the thickness, texture, and elasticity of the skin, the intensity of wrinkles and furrows. Overexposure to the sun will often produce more wrinkles and face plastic will have to be combined with a laser resurfacing to eliminate them. The choice of incision site will depend on the hair implant line. Another procedure commonly associated with facial plastic surgery is nose rejuvenation (see Rhinoplasty). Smokers should decrease or even stop before surgery. You should avoid taking aspirin, which can cause bruising. Drains can be used to prevent fluid buildup. Recovery varies greatly. Sleeping with your head elevated helps decrease swelling and bruising and drains are removed in one to two days. Temporary asymmetry is normal and can be caused by swelling and bruising. The bruising should disappear within two to three weeks. Corrective makeup can be applied within a few days. The numbness can last several weeks. Sun exposure is only allowed after the first month. It takes several weeks to evaluate the new look. Some men may have to shave behind their ears where shaved skin goes.

Duration of Results:
Usually 7 to 12 years.

Rhinoplasty is the name of plastic surgery on the nose. This surgery is performed on the nasal structure in order to improve the aesthetics or breathing of the individual. It serves to correct traumatic or natural deformities and also to correct dysfunctions. Often Nose Plastic Surgery is associated with a Mentoplasty so that the result produces a greater harmony with the face shapes (Profileoplasty).

Duration:
Plastic surgery on the nose usually takes 1-2 hours, although, in more delicate procedures it may take a little longer.

Anesthesia:
May be local or general.

Details:
In plastic surgery on the nose, the skin of the nose is separated from the bone and cartilage structure, which is then sculpted into the desired shape. The nature of the sculpture will depend on your problem and the surgeon’s preferred technique, sometimes we perform rhinoplasty inside the nose (closed), sometimes and increasingly often opt for an “open” procedure, especially in more complicated or complicated cases. secondary rhinoplasty.

Finally, the skin is readjusted over the new osteo-cartilaginous outline, and a splint is applied to help the nose maintain its new shape. Internal nasal plugs are also used.

Plastic surgery in the nose is advised only to patients over 18 years old, except in case of respiratory complications or congenital malformations. Prior to this age Plastic nose surgery is not indicated because facial structures change with growth and it is advisable to wait for definitive development before undergoing corrective surgery.

Tummy tuck is a plastic surgery of the abdomen for functional or aesthetic correction of the abdominal wall. Abdominoplasty, or abdominal dermolipectomy, aims to correct excess tissue, whether fat or skin, in the abdominal region. It may be recommended for women who have had many children, as distension of the abdomen during pregnancy can cause excess abdominal tissue and distention to the sides of the rectus abdominis muscles.

For the same reason, tummy tuck can be recommended for people who were obese and lost a lot of weight leaving excess tissue in the abdominal region. Tummy tuck may also have application to correct deposit of fatty tissue in the abdominal region which causes a non-aesthetic protrusion.

What is the difference between tummy tuck and liposuction? While liposuction is used for cases of excess fat, abdominoplasty (abdominal dermolipectomy) is a more complex surgical intervention that allows major changes. Although abdominoplasty allows weight loss, its main purpose is aesthetic, not the elimination of pounds, with good results can be achieved with little tissue removal.

Hospitalization:
48 hours with drains, brace use for 2 months, prevention of thromboembolism and rest to avoid seromas is recommended.

Liposuction is a surgery to reduce the volume of body fat in localized areas, giving the patient a better body contour. Although many think, liposuction is not meant to lose weight, because the biggest change is in body silhouette rather than balance. The best results are obtained in Liposuction where the patient has localized fat. Surgeries in large areas and large volumes are more likely to leave irregularities.

Anesthesia Type:
It depends on the area to be operated on and the volume of fat to be aspirated. It can be from local anesthesia, local sedation, epidural, or general.

Length of stay:
Varies from 12 to 24 hours, depending on the size of liposuction.

Postoperative:
There is usually edema (swelling) and bruising (purple spots), which resolve on their own within 21 days, in most cases. During the first month after liposuction, the patient has noticeable improvement in edema, when he then begins to notice hardening in the operated area, due to internal healing. This hardening progressively improves during the 2nd and 3rd months, at which time the skin shrinks further. An elastic brace should be worn for 45 days and lymphatic drainage and ultrasound indicated 7 days after surgery to accelerate recovery, reduce swelling and hardening.

Complications:
Complications in liposuction are rare, but can be cited: hematoma, seroma, irregularities, infection, thrombosis, accidents during surgery and anesthetic problems. Seroma, the most common complication in large liposuction, is the accumulation of clear fluid in the operated region, forming as a “water bag”. In these cases, emptying is performed through punctures, with a resolution of the problem and without prejudice to the result.

Definitive Result:
The definitive result of liposuction is after 6 months.

Narrowing or tightening of the vagina (vaginoplasty) is a correction most frequently requested in the last 15 years, especially by the young woman after the first or second delivery, where it is evident from her clinical report that sexual intercourse is changed profoundly by the laxity of their tissues. This careful reconstruction of the vaginal canal is performed at the level of the relaxed muscles of the pelvis (perineum pavement muscles) and the modeling of the vaginal mucosa is performed, giving a three-dimensional structure, balanced and harmonious, with notorious functional sexual repercussion. which leads patients to state “intimate relationship has improved dramatically or returned to pre-birth situation”.

Hymen Reconstruction
The reconstruction of the hymen is a surgery, in comparison to the others, rarer, and its justification in social, cultural and religious factors, being lived in some individual cases with intense “stress”, being almost in danger the own life of the young, that attribute or define as “family honor.”

Highlight: “The most common surgical correction is pubic liposculpture (Mount of Venus).

Big Lip Hypertrophy (bat wings).

Excess tissue excision of the large lips usually performed with local anesthesia and sedation without hospitalization.

Otoplasty aims to correct aesthetic imperfections of the ears such as the so-called flappy ears.

This type of defect is quite common and has very negative psychological effects, especially in children. Because of this, the ideal age for otoplasty is 5 to 7 years, when the ears are already formed and practically their final size.

Preoperative
Aspirin or any acetyl salicylic acid-based formula should not be used for at least ten days before surgery. The ears and hair should be washed thoroughly on the eve of the surgery. If long, the hair should not be cut before the operation as it will help to disguise in the early days.

Surgical procedure
An incision is made behind the ear where excess skin is removed and cartilage is shaped or removed to correct any defective contours. The cartilage is then sutured to a position within normal standards.

Otoplasty is a technically simple surgery but requires a thorough work. The scars are almost invisible, located in the groove between the ear and the mastoid. In general, local anesthesia associated with a mild sedative is used.

Anesthesia:
General anesthesia may be recommended in some cases.

Postoperative
Postoperative pain may occur after otoplasty. If it occurs, a common painkiller solves the problem. A dressing held for a period of 5 to 7 days is placed over the ears during which one should avoid lying on his side. After removing the dressing, the patient should wear a sleeping cap for at least one month. Stitches are removed on the 10th or 12th day after surgery. It is recommended to avoid prolonged sun exposure in the first month.

Definitive Result:
Although the immediate effect is satisfactory, the definitive result of ear plastic surgery can only be seen around the 6th month.

Many interventions in plastic surgery have as a consequence the existence of scars that depend more on biological factors than on the competence of the surgeon.
Indications:

  • Widened scars;
  • Hypertrophic scars (elevated, reddish and with pruritis);
  • High keloid scar invading the neighboring tissues that grows behaving like a tumour.

The surgeon’s goal is to replace the scar with another one with better quality and as imperceptible as possible.

Anesthesia
The procedure is performed almost always under local anestesia.

Also called thigh lifting (cruroplasty) and it has indication after accentuated weight loss allowing simultaneous excision of fat and skin tissue.
It´s important to quit smoking since smoking (as in other interventions in which is required large skin displacements) exponentially increases the complications.

Inpatient care:  1- 2 days.

Duration:  2h of surgery.

Contraindications:

  • Lymphedema
  • Diabetes
  • Arterial or venous insufficiency

It should be noted that as a final result there will always be a scar along the inguinal fold that will depend not only on the technique but, above all, on the biological characteristics of the individual.
